Secondary Placement of Akreos AO60 Transscleral-Sutured Lens and McCannel Suture Iris Repair

This patient previously experienced ruptured globe injury with iris prolapse and expulsion of her intraocular lens implant after her superior extracapsular cataract incision dehisced. Following wound repair and later suture removal, a secondary Bausch + Lomb Akreos AO60 intraocular lens implant was placed using a Gore-Tex transscleral-sutured method. She experienced photophobia due to persistent mydriasis after the injury so an iris repair was performed using a McCannel technique.

The Akreos AO60 lens is made of foldable hydrophilic acrylic, so it may be placed through a 3.5-4 mm clear corneal incision. In this case, it was decided to place the lens through a superior incision to relax some of the superior corneal steepening that resulted from her previous injury and repair. When suturing the lens, the 4-point fixation of the Akreos AO60 may enhance stability and protect against lens tilt compared to lenses with 2 eyelets.

Hydrophilic acrylic is known to develop lens opacities with prolonged exposure to air or gas. This is a consideration when placing the lens during a combined procedure requiring gas or air bubble placement within the eye.
