Cutaneous lacrimal fistula

This is Richard Allen at the University of Iowa. This video demonstrates a cutaneous fistula of the lacrimal sac. The patient has a history of a congenital nasolacrimal duct obstruction. Fluorescein is irrigated is through the lower punctum and canaliculus. As is noted, there is egress of fluid medially through the fistula. In addition, there is reflux into the eye through the other punctum demonstrating the NLDO. I think it is very important to inspect the medial canthus of all congenital nasolacrimal duct obstruction patients carefully to ensure that there is no cutaneous fistula
