In checking distance visual acuity, the patient should wear their glasses if they have any. An occluder is used so that one eye is checked at a time. One should start with a 20/40 target and then have the patient read the smallest line they can see. The patient is then instructed to look through the pinhole of the occluder to see if smaller lines can be discerned. The opposite eye is then checked in the same manner. Improvement in vision with the pinhole usually indicates a refractive error or media opacity.
