Revision of pillar tarsorrhaphy

This is Richard Allen at the University of Iowa. This video demonstrates revision of a medial pillar tarsorrhaphy. The pillar tarsorrhaphy is demonstrated. It is a bit too loose and is not resulting in adequate closure of the eye. Therefore, Westcott scissors are used to transect the pillar at its attachment to the lower lid. The conjunctiva of the lower lid is excised in the area of the attachment with Westcott scissors. The pillar is then inspected and shortened an appropriate amount. The pillar is then engaged with a double-armed 5–0 Vicryl suture. Each arm of the suture is then placed through the eyelid, full-thickness, entering at the area of the resected conjunctiva. The sutures are then tied over a cotton bolster. The patient will return in one week for removal of the suture and reevaluation. As is noted, closure of the eyelid is improved.
