Knife Too Deep: slit knife into lens

In this case, a junior resident (with about 10 cases of experience) was attempting a clear corneal incision and had poor control of fixation with the Fine-Thornton fixation ring. It also appears that the steel blade was dull. Too much force was needed, and the slit knife slipped into the lens, which is a rare complication.The rhexis was continued with forceps, but had gone radial in one direction. Nevertheless the case went well.
