This video demonstrates my standard technique for iStent insertion, along with a discussion of how we can target areas of increased pigmentation of the trabecular meshwork to increase the effectiveness of pressure lowering with iStent. If trabecular meshwork pigmentation is patchy, increased pigmentation can be used as a guide (as in this video). If blood reflux into Schlemm's canal is segmental, areas of increased blood reflux could also be targeted.
In this particular patient, the post-operative month 1 pressure was in the low teens without any medications.
