A 74-year-old man presented for evaluation of narrow angle glaucoma in both eyes. On exam there was notable iridoschisis of the inferior iris in both eyes. This is a rare condition in which the anterior iris stroma separates from the deeper tissues. The fragments of iris can rub on the cornea and result in pigment deposition and corneal decompensation. Glaucoma can develop from an angle closure mechanism as a result of pupillary block or from an open angle mechanism as pigment is liberated into the trabecular meshwork.
