A 72-year-old man presented one week after uncomplicated cataract surgery for progressive photophobia and blurry vision in the surgical eye. On exam, there was a retained lens fragment in the anterior chamber. Small pieces of nuclear or cortical lens material can sometimes be hidden in the ciliary sulcus at the end of cataract surgery. Patients present with an inflammatory picture. Often times cortical material can be observed and treated medically with anti-inflammatory topical treatment. Nuclear material, as in this case, however, often requires surgical intervention with removal of the lenticular remnant.
