O to Z Flap

This is Richard Allen at the University of Iowa.  This video demonstrates the use of an O to Z flap.  The patient had a basal cell carcinoma removed from the left cheek using the Mohs technique.  The flaps are planned and then a number 15 blade is used to make an incision through the skin followed by undermining of the flaps with Westcott scissors.  Transposition of the flaps then results in adequate closure of the defect without tension.  This patient also has a defect of the left lower eyelid that will be repaired later.  The flaps are then sutured together with deep interrupted 5-0 Vicryl sutures to produce the Z.  The skin is then closed externally with interrupted 6-0 prolene sutures placed in a vertical mattress fashion.
