Right Medial Orbital Decompression
This is Richard Allen at the University of Iowa. This video demonstrates a transcaruncular incision for approach to the right medial orbital wall in a medial orbital decompression.
Westcott scissor are used to make an incision between the caruncle and the plica.
Steven's scissors are then used to bluntly dissect toward the area just posterior to the posterior lacrimal crest.
A Desmarres retractor can then be placed to provide traction.
A malleable retractor is then placed to visualize the medial orbital wall.
The Freer periosteal elevator is then used to make an incision through the periosteum and elevate the periosteum along the medial orbital wall.
The initial step is to identify the anterior ethmoidal neurovascular bundle.
