Upper Eyelid Crease Anterior Orbitotomy

This is Richard Allen at the University of Iowa. This video demonstrates an anterior orbitotomy through an upper eyelid crease incision.
The patient was found to have a mass at the level of the preaponeuorotic fat, centrally.
An incision is made with the 15 blade through the skin.
The monopolar cautery is then used to dissect through the orbicularis muscle and expose the orbital septum.
Additional dissection is performed between the orbicularis muscle and the orbital septum.
Westcott scissors are then used to dissect through the orbital septum to expose the preaponeurotic fat.
Blunt dissection exposes the mass which had the gross appearance of lymphoma, which was confirmed by the pathologist.
Westcott scissors are used to biopsy the mass, and the mass is sent to immunopathology as well as light microscopy.
Hemostasis is obtained with the unipolar cautery.
The lid crease incision is then closed with interrupted 6-0 Prolene suture.
Antibiotic ointment is placed over the incision and the patient returns in approximately one week.
