Q18: What is the most likely cause of the combined iris and lens findings seen in this patient?

A18: A prior episode of acute angle closure glaucoma.  The small anterior subcapsular opacities are called glaukomflecken, which are caused by lens epithelial cell necrosis during episodes of angle closure glaucoma. The iris atrophy is also secondary to elevated intraocular pressure during episodes of angle closure glaucoma.
